ABOUT THE SERVICE

This department is interconnected with the billing department in settling the
account of the patient prior to discharge. Its primary concern is to assist
those people in availing their Philhealth benefits therefore helping them
process their Philhealth concerns with ease and speed. This department is
composed of seven (7) administrative officers and administrative aides.

SCHEDULE OF AVAILABILITY OF SERVICE:

· Weekdays: 8:00 o’clock am – 5:00 o’clock pm
· Weekends: 8:00 o’clock am – 4:00 o’clock pm

WHO MAY AVAIL OF THE SERVICE?

· Patient is a qualified member.
· Patient is the spouse of a qualified member.
· Patient is a qualified member’s child age 21 years old and below.
· Patient is a qualified member’s parent age 60 years old and above.

WHAT ARE THE REQUIREMENTS?

· Sponsored/ Philhealth ng Masa:
1. Patient is a qualified member:
a. Family Health Card/Member Data Record (MDR)
b. Birth Certificate
2. Patient is the spouse of the member:
a. Photocopy of Marriage Contract/Certificate with registry number.
3. Patient is member’s child age 21 years old and below:
a. Photocopy of birth certificate with registry number/Baptismal
Certificate with the name of the parent appearing in the
certificate.
4. Patient is member’s parent age 60 years old and above:
a. Photocopy of dependent parent’s birth certificate with registry
number.
b. Or any of the following: Passport/Driver’s license/Senior Citizen
ID with the patient’s date of birth indicated in the said documents
provided that they submit a notarized affidavit signed by two
people not related by blood testifying to the patient’s date of birth
including a certification from the National Statistics Office (NSO)
or Local Civil Registrar (LCR) that no record has been found.

· Employed Members


1. Patient is a qualified member:
a. Family Health Card/Member Data Record (MDR)
2. Patient is the spouse of the member:
a. Photocopy of Marriage Contract/Certificate with registry number.
3. Patient is member’s child age 21 years old and below:
a. Photocopy of birth certificate with registry number/Baptismal
Certificate with the name of the parent appearing in the
certificate.
4. Patient is member’s parent age 60 years old and above:
a. Photocopy of dependent parent’s birth certificate with registry
number.
b. Or any of the following: Passport/Driver’s license/Senior Citizen
ID with the patient’s date of birth indicated in the said documents
provided that they submit a notarized affidavit signed by two 
people not related by blood testifying to the patient’s date of birth
including a certification from the National Statistics Office (NSO)
or Local Civil Registrar (LCR) that no record has been found.

· Individual paying Members and Overseas Filipino Worker: Attach a copy of
premium payments or copy of receipts of payments made for the last
three/six/nine/twelve months before patient’s confinement including the
following:
1. Patient is a qualified member:
a. Family Health Card/Member Data Record (MDR)
2. Patient is the spouse of the member:
a. Photocopy of Marriage Contract/Certificate with registry number.
3. Patient is member’s child age 21 years old and below:
a. Photocopy of birth certificate with registry number/Baptismal
Certificate with the name of the parent appearing in the
certificate.
4. Patient is member’s parent age 60 years old and above:
a. Photocopy of dependent parent’s birth certificate with registry
number.
b. Or any of the following: Passport/Driver’s license/Senior Citizen
ID with the patient’s date of birth indicated in the said documents
provided that they submit a notarized affidavit signed by two
people not related by blood testifying to the patient’s date of birth

· Lifetime Members: A Copy of Philhealth Lifetime Member ID including any of
the following:
1. Patient is a qualified member:
a. Family Health Card/Member Data Record (MDR)
2. Patient is the spouse of the member:
a. Photocopy of Marriage Contract/Certificate with registry number.
3. Patient is member’s child age 21 years old and below:
a. Photocopy of birth certificate with registry number/Baptismal
Certificate with the name of the parent appearing in the
certificate.
4. Patient is member’s parent age 60 years old and above:
a. Photocopy of dependent parent’s birth certificate with registry
number.
b. Or any of the following: Passport/Driver’s license/Senior Citizen
ID with the patient’s date of birth indicated in the said documents
provided that they submit a notarized affidavit signed by two
